RHEUMATOID ARTHRITIS (RA)
Unlike the pathophysiology of OA (Osteoarthritis) which is largely mechanical, RA is an autoimmune condition. It is characterized by inflammation and occurs usually in symmetrical parts of the body. The clinical manifestations are extremely variable and display exacerbation and remission. RA is accompanied by systemic features such as morning stiffness and fever. It is not entirely clear why immune diseases develop. The initial triggers of RA can include a combination of factors such as hormones, genetics, environmental factors, bacterial overgrowth in the digestive tract, increased gut permeability and retention of toxic metabolites. Once the initial immune response is triggered, cells of the immune system produce auto antibodies and inflammatory cytokines. Cytokines are a broad and loose category of small proteins that are important in cell signaling. They are released by cells and affect the behavior of other cells. Cytokines are crucial for fighting off infections and other immune responses. However, they become deregulated and pathological in inflammation, trauma and sepsis.
Pathophysiology of Rheumatoid Arthritis
The auto antibodies and inflammatory cytokines create an inflammation cascade resulting in the formation of pannus. Pannus is an abnormal layer of fibrovasula or granulation tissue and it invades and destroys cartilage and bone. Additional joint damage and systemic complications ensue resulting from a complex process of inflammatory mediators being released in the affect joint.
Labeled “the nation’s primary crippler” it occurs as a result of inflammation and weakening of the joint capsule and the cartilage lining the joints. It can be classed as either primary (idiopathic) meaning without discernable relation to another disease. It mainly presents in middle age or elderly individuals. Secondary OA may present at any age in a joint due to trauma, disease such as TB (tuberculosis), gout, rheumatoid arthritis or deformity such as congenital dislocation of the hip. OA is often asymptomatic but joint pain, decreased range of motion, crepitus (a crackling sensation when the joint is moved) and joint enlargement are prominent symptoms. Other less prominent symptoms may include stiffness, redness, swelling and fatigue. A chief factor in the development of this form of arthritis is poor circulation to the microscopic blood vessels that carry oxygen and other nutrients to the joints. The standard American diet creates fatty streaks and plaques to occur in the blood vessels. This diminishes the diameter of the vessel. The aggregation (sticking together) of red blood cells following a fatty meal impedes blood flow and decreases oxygenation of the joints.
The main pathological features of OA include injury and loss of the articular cartilage. These alter the shape of the articular surfaces. The earliest gross changes are softening of the joints and/or stiffness of the articular cartilage. This is brought about by unmasking and fragmentation of the collagen fibril structure of the cartilage. This change is called cartilage fibrillation. Microscopically, the fibrillated cartilage shows clefts or fissures which begin in the superficial layers. They then deepen as the cartilage is eroded and worn away from the surface by tangential flaking, pitting and grooving.
Reduced oxygenation of the joints interferes with the normal repair and remodeling of bone that is ongoing throughout life. In this environment of nutritional inadequacy, tiny structural imperfections that have developed progress to a state of chronic degeneration.

Physiological Effects of Endermologie™
The mechanism of action was scientifically studied at UCLA School of Medicine using both human and animal subjects. The SUS scorfa swine model was studied by using subcutaneous endoscopy, photography, gross tissue examination and histological examination. The human subjects were studied by using lymphoscintigraphy, skin perfusion venous color-flow Doppler ultrasonography and laser Doppler blood flow analysis of skin perfusion.
Laser Doppler measurements showed a 4-fold increase in cutaneous blood flow. This increase peaked 10 mins post treatment and lasted for more than 6 hours.
Color-flow Doppler measurements showed increases in the flow velocities of the subcutaneous veins within the adipose tissue and a concomitant decrease in flow velocities of the deep muscular veins.
The conclusion in this study was that, Endermologie™ produces a profound physiological alteration in cutaneous perfusion, subcutaneous perfusion and lymphatic flow. This effect seems to be delayed and prolonged lasting for hours after completion of the treatment.
